How to set up a log-in with our Patient Portal.

Click here to access the portal.

Click on "Using Mobile Phone" button and enter your name as written on your driver license.  If you get an error, your name may be all in CAPS in our system.  If you still have further issues please call us at (954) 491-6200 to confirm how your name is written in our system.

Once a password is created, you will log in using "User Credentials" which will be your e-mail address and the password you created.

Healow Access

Download the Healow App

You may download the Healow App on your iPhone or Android device.

Request Refills

Don't wait for your pharmacy to reach out to us once you are out of refills, send us a request before you run out.

Log into the Patient Portal and click on "Messages" and "Refills Request"

Join A TeleVisit 

Open the website on any device that has audio/video capability.

The Medical Assistant will provide you with a 12-digit code.

Medical History Form for New Patients Only

Save time filling out your medical history prior to your appointments.

Log into the Patient Portal and click on "QUESTIONNAIRES" Click on "Medical History" and also on "Surgical and Allergies"

When you arrive to your appointment, your medical history will already be in our EMR.

Please be sure to get any labs or imaging done at least one week prior to your appointment.

If you cannot make it to your appointment, contact us and we can change it to a TeleVisit. (Audio/Video is required)

Please keep in mind there is a no-show fee.
